We are excited to offer free health services as part of a collaboration between the Community Health Services at MNLC, and Black Creek Community Health Centre.

Who Can Access Services?

🔹 OHIP card holders without a health care provider

🔹 Non-insured individuals

🔹  Refugee claimants with IFHP coverage

All Services Are 100% FREE!

You can now reach us through our new program emails below:

Our office hours remain the same: You can drop in at our Keele location (2737 Keele St, units 9, 111, 112, North York) or Finch location (1122 Finch Ave West, Unit 1, North York):

  • Settlement staff are available onsite at 2737 Keele St Units 111 & 112 from 9:00 AM to 3:00 PM Monday to Fridays to answer questions and provide guidance. Wi-fi is now available.
  • Community Health Services are available at 1122 Finch W Unit 1 on Wednesdays from 9:30 am to 3:00 pm. The HAP clinic is closed for the summer but our Health Navigator can provide guidance in accessing health services.
  • Mental Health and BREM staff are available by appointment only.
